The section from Peso da Régua to Pinhão is the one you see on every brochure: steep schist slopes planted with rows of vines that drop straight to the water's edge. Pinhão to Pocinho: The Wild Section
Beyond Pinhão, the line changes character. The valley narrows. The track hugs the Douro so closely that on the left side of the train going east, you are looking straight down into the river. The vineyards continue but the human presence thins out. Fewer quintas are visible from the rails and more sections of bare gorge appear. The train stops at small stations that feel abandoned, though they are not. The population density drops. The tourists on the train drop even faster.
Pocinho is not a destination in the usual sense. It is a village with a hydroelectric dam and the end of the electrified line. There is a cafe near the station. Get off, stretch, take a photo of the sign that says you have reached the end, and wait for the return train. The value is in the ride back. Coming west from Pocinho to Pinhão, the right side of the train faces the river for the entire scenic segment, and you see the gorge from the opposite direction with different light.

For the best river views on the eastbound journey, sit on the left side of the train from Peso da Régua onward. On the return, sit on the right side from Pinhão back to Porto. The train has no catering carriage. None. Bring water and food for the day. There is no shop on the platform at Pocinho and limited options at Pinhão. Pack a sandwich, fill a water bottle at your hotel, and consider a thermos of coffee.

That combination covers a round trip by rail from Porto to Pinhão or Pocinho plus a boat segment on the river. If you want the steam option, CP runs the Comboio Histórico do Douro from June to October on Saturdays and Sundays plus some public holidays. The Honest Caveat on the Pinhão to Pocinho Stretch
The section beyond Pinhão is worth the extra time if you value empty trains, a wilder gorge, and the sense of having reached the end of a line rather than just the end of a tour. But it is not a prettier landscape in any absolute sense. The vineyards around Pinhão are more photogenic, more cultivated, and more obviously the Douro Valley you came to see. The stretch to Pocinho is the same river in a less manicured setting. The value is its quietness and its completeness. If your tolerance for sitting on a regional train for seven hours round trip is low, stop at Pinhão. If you want to say you rode the line to its terminus and you are willing to bring your own lunch, keep going.
The thing that goes wrong most often is that travellers underestimate how long the return feels. The train does not move faster on the way back. Three and a half hours each way is a full commuter day. Bring a book. Charge your phone. Do not expect the conductor to sell you a sandwich, because they will not.
